By submitting this application, I agree that EQUAL-ARK Singapore Ltd (EQUAL) may collect, use and disclose my personal data for the following purposes in accordance with the Personal Data Protection Act 2012:
- processing of and assessing the membership application,
- administration of the membership within EQUAL
- verifying my identity;
- providing me with information on EQUAL’s events or activities;
administering, managing and operating the activities of EQUAL, including generating publicity for EQUAL and its events (such as using photographs, videos or other media containing my images taken during EQUAL events);
- responding to, handling, and processing queries, requests, applications and feedback from me;
- complying with any applicable laws, regulations, codes of practice, guidelines, or rules, or to assist in law enforcement and investigations conducted by any governmental and/or regulatory authority;
- any other purposes for which I have provided the information;
- transmitting to any unaffiliated third parties including EQUAL’s third-party service providers and agents, and relevant government and/or regulatory authorities, whether in Singapore or abroad, for the above mentioned purposes; and/or
any other incidental purposes related to or in connection with the above.
I agree that if I provide any personal data relating to a third party (such as my spouse, children, parents or partner), by submitting such information to EQUAL, I confirm that I have obtained the consent of the third party to provide EQUAL with their personal data for the relevant purposes (such as emergency contacts) and for their personal data to be retained, used or disclosed by EQUAL accordingly.
I confirm that all data submitted to EQUAL, personal or otherwise, including personal data of third parties (such as my spouse, children, parents or partners) is complete, accurate, true and correct. EQUAL may reject any application for any reason, including inaccuracy in the data provided to it.
